Hello, thank you for your interest in receiving the COVID-19 vaccine at CNU. You will be required to fill out a pre-screening survey to determine your eligibility to schedule an appointment at our site.
Upon completing this pre-screening survey, you will be invited to schedule an appointment on the Calvax site using provided links. Please keep in mind that you will be required to answer similar questions on Calvax; however, your responses on Calvax WILL NOT be used for eligibility screening. For your safety, please answer all of the questions on the CNU pre-screener as honestly and to the best of your knowledge as possible.
***Please arrive to clinic check-in no earlier than 15 minutes before your scheduled appointment. Patients will only be allowed to line up 15 minutes before their scheduled appointment. Patients may wait at their vehicle in the CNU parking lot until their appointment time.***
***Clinic check-in begins outdoors. Please wear weather appropriate clothing to your appointment. Please be prepared to easily bare your arms for injection.***
***Please be available to schedule a second appointment 21 days after your first dose. If you are not available for a second appointment 21 days after your first dose, please do not schedule an appointment with CNU at this time. We invite you to schedule with us when you are available for both a first and second appointment 21 days apart.***
